Full Job Description

Full UK manual driving licence is desirable but not essential

You will be a friendly face to someone who may be struggling to do things for themselves due to age, illness, or vulnerability. You will assess their needs and help them to remain in their own homes, preventing an avoidable hospital admission. If you have a caring, patient, and helpful nature you could make a tangible difference to someone's life. You'll allow them to enjoy healthier, more fulfilling independent lives. This is not just a job, but an amazing way to give back to your community!

What will a day in the life of a Community Support Worker involve?

  • Providing reactive, focused, and flexible support to people following a hospital stay or who have been identified at risk of decline within the community.

  • Assessing service users' needs, completing and following a support plan for them.

  • Assisting with personal care including bathing, dressing, and medication.

  • Providing practical and emotional assistance (telephone calls, welfare checks etc).

  • Providing guidance to access support within the community or help to attend medical appointments.

  • Responding to referrals from Clinicians and Community Health Professionals.

    Good knowledge of services provided by the NHS and Social Care.

  • Experience of working with vulnerable people perhaps with physical conditions or mental health issues.

  • Experience of delivering excellent service to the public.

  • Ability to deliver clear written and verbal communications to service users and external organisations.
