We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.

This is a part-time permanent post of 29.6 hours per week, with a shift pattern Monday to Friday.

Please note the salary is Pro Rata for part time hours. 

Are you a compassionate and dedicated nurse looking to make a real difference in the community? We are seeking a Community Staff Nurse (Band 5) to join our Focused Intervention Team (FIT) within West Dunbartonshire Health and Social Care Partnership.

About the Role:

As a Band 5 Nurse within FIT, you will be clinically responsible for a caseload of patients, working closely with the team to ensure individuals can safely remain at home, avoiding unnecessary hospital admissions. You will play a key role in delivering high-quality care, supporting the patient's recovery and independence in their own home environment.

Key Responsibilities:

  • Manage a caseload of patients, providing care and interventions to enable them to stay at home as a safe alternative to hospital care.
  • Work collaboratively with the COPD Nurse (Band 6) and Care Home Liaison Nurse (Band 6), offering support when necessary.
  • Provide cover for the Band 6 nurses during absences or periods of high workload to ensure seamless care delivery.
  • Contribute to multidisciplinary team working, ensuring integrated care for patients.

Essential Skills and Qualifications:

  • Registered Nurse with a valid NMC Pin.
  • Experience in a community nursing setting or similar role is desirable.
  • Strong clinical decision-making and assessment skills.
  • Ability to work independently and as part of a multidisciplinary team.
