Hospital at Home and Frailty
Are you a compassionate and skilled Consultant looking to make a meaningful impact on patient care? We are seeking a Hospital at Home Consultant to join our dynamic frailty team.
As a Hospital at Home Consultant, you will be responsible for overseeing the provision of acute medical care to patients in their own homes. You will lead a multidisciplinary team of healthcare professionals to deliver high-quality, patient-centred care in a home setting. This role requires a strong clinical background, excellent communication skills, and the ability to adapt to various patient needs.
Key Responsibilities:
- Provide clinical leadership and expertise in the management of acute medical conditions in the home setting.
- Work within our Older Adult Assessment Unit and provide support to the Emergency Department and Assessment areas, conducting comprehensive assessments of patients to determine suitability for Hospital at Home care.
- Develop and implement individualised care plans in collaboration with the patient, family members, and other healthcare professionals.
- Monitor patient progress, adjust treatment plans as needed, and ensure timely communication with referring physicians and other stakeholders.
This is an opportunity to supervise and support the Hospital at Home team, including nurses, therapists, and support staff.
You will play a pivotal leadership role in setting up the Hospital at Home Service in Clyde and ensure compliance with clinical protocols, quality standards, and Participate in continuous quality improvement initiatives research to enhance the delivery of Hospital at Home services.
