What we do: 

The Social Care Worker – Rehabilitation Support is a key part of the Intermediate Care Team.   The Intermediate Care Team is a service delivered jointly by NHS Shetland and Shetland Islands Council (SIC).  The team is made up of: a Nurse Team Manager, Occupational Therapists, a Social Care Worker Practitioner – Rehabilitation Support and Social Care Workers – Rehabilitation Support.  Referrals into the team are made via a professional involved in the individuals care, for example, social work staff, a GP, Community nurse, or Physiotherapist. 

The Intermediate Care Team provides a short term support and reablement service to increase an individual’s independence, reduce their stay in hospital or in a care home, and help avoid an unnecessary hospital stay. The team supports the Service User to identify achievable and realistic (SMART) goals and provides the support an individual needs to work towards those goals. The team also provides a Community Multifactorial Falls Risk Screening service.

What you'll do be doing: 

As a Social Care Worker – Rehabilitation Support you will actively participate with the multidisciplinary team, service user and their families/carers, in a team approach to planning, development, implementation and evaluation of service user rehabilitation, in a variety of settings, including their own homes.

  • Salary shown in the advert is inclusive of Distant Islands Allowance of £2,742 per annum (pro rata)
  • Temporary Full Time post for 37.0 Hours per Week , 52.18 weeks per year. Temporary contract for a period of approximately 12 months. This is for the duration of a period of maternity leave, and will terminate following the return to work or resignation of the postholder.
  • Preferred candidates will be required to join the Protecting Vulnerable Groups (PVG) Scheme or undergo a PVG Scheme update prior to a formal offer of employment being made
  • The successful candidate will have to complete registration with the Scottish Social Services Council (SSSC) within 6 months of the agreed start date
  • A relocation package does not apply to this post
  • We will pay interview expenses from within Shetland
  • You must have the ability to travel throughout Shetland at short notice and where public transport is limited
