The aim of Social Work Resources is to promote social welfare and provide effective care and support to meet the needs of vulnerable people in South Lanarkshire.

In partnership with community planning partners, voluntary and independent provider organisations, we offer a range of services designed to enable, support, improve and protect the health and social care of people using our services.

An opportunity has arisen for a Social Work Assistant within our Self-Directed Support Review Team. The SDS Review Team are a specialist team, reviewing SDS packages for both supported people and carers, to achieve best personal outcomes. You will be working with adults (18+) who will have a range of health and social care needs within community setting. 

You will be responsible for the review and subsequent completion of an updated assessment of risks and eligible needs, developing support plans to meet identified needs and outcomes under the supervision of the Team Leader. The successful candidates will be responsible for liaising with a variety of partners, including supported people, their families and unpaid carers, as well as care providers and commissioned services. The successful candidate will demonstrate an ability to relate to people of all ages and backgrounds and be able to gain their trust, in order to help them get the best from our services. They will be able to deal with a variety of situations where tact, patience and understanding will be needed, along with the ability to assess and cope with these situations in a sensitive and non-judgmental way. It is a requirement of this post that you have practical experience of working with various IT systems.

Shortlisting will take place on Friday 4th October 2024 and in-person interviews will take place on Friday 18th October 2024.

Hours: 35 hour post

Salary: Grade 2 Level 4, £33,797.15 - £34,764.35
