Full Job Description

This post would offer an opportunity to work as part of the newly developed home team to support patients at home and aid admission prevention and discharge without delay.

  • Can you demonstrate your skills through re-treatment programs, supporting self-management, support mental well-being and maximising independence?
  • Can you confidently self-coordinate a caseload whilst proactively seeking shared best decisions with individuals and multi-disciplinary multi-agency partners? Can you demonstrate effective working relationships across Locality Teams inclusive of GPs, Nursing, Social Work, Social Care, AHPs, Third and Independent sector organisations?
  • Are you aware of Home Teams and how you will be contributing towards working differently in the future to achieve transformational improvements for the health and well-being outcomes for the people of Dumfries and Galloway?
