Fixed term: 18 months

The purpose of the post: 

  • As part of the Discharge Hub Team, responsible for the management of the identification of patients, in liaison with Discharge Hub Manager, medical and nursing staff, for a discharge/transfer pathway from other hospitals in Lothian ensuring optimum provision of appropriate beds within EHSCP, including admissions from community when required.
  • To manage the Home First review process on behalf of the Partnershipincluding daily management of waiting lists, identification of appropriate bed, and coding of patients is key to ensuring this process is effective and responsive to daily demands on patient flow.
  • To manage the daily identification of delayed discharges for the Partnership with specific responsibility for coding patients under ISD National Data Recording practices.
  • To provide a full and effective comprehensive range of administrative support which will enable the department to fulfil its function to a high quality standard and meet the demands of daily patient flow across Lothian. This will include preparing statistical information for the Discharge Hub Manager/EHSCP/Scottish Government using appropriate systems.

NHS Scotland is reducing their full time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay.  This reduction will also be applied pro rata for part time staff.  This advert and any subsequent offer/contract of employment therefore reflects the new working hours.  However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.  If you have any questions or concerns please contact the Recruiting Board.
