Short Term Community Rehabilitation - Specialist Occupational Therapist - Stewartry Home Team
Dumfries and Galloway Health and Social Care Partnership (HSCP) is undergoing a period of transformative change to develop multi disciplinary integrated teams.
The Home Teams will deliver person centred care with the promotion of independence at the core of service delivery.
We are looking to welcome enthusiastic registered Occupational therapists with a keen interest in working within an adult community health setting. The focus for this post is provision of occupational therapy assessment and interventions to help maximise client function and independence, focussing predominantly on short term rehabilitation, proactive and preventative interventions.
You will be expected to expertly manage a caseload to provide a full service of assessment, appropriate management, onward referral and discharge to a range of patients referred to the service. You will work within multi-professional teams to ensure co-ordinated care for patients.
There is a focus on utilising professional skills to deliver rehabilitation and enablement
