An exciting opportunity has arisen for a Band 4 AHP Assistant Practitioner to join the Upper Annandale Home Team.

This post would offer an opportunity to support the both Occupational Therapy and Physiotherapy interventions by delivering a plan of rehabilitation programmes and supports under the supervision of a registered practitioner.

The post holder will be required to work autonomously, managing a self coordinated caseload whilst proactively seeking shared decision making with individuals and the wider multi-disciplinary Home Team.

A current driving licence is an essential due to the rural geographical area this team covers. However, if you have a disability which precludes you from holding a driving licence, consideration will be given if alternative transport can be arranged.

Although the post is advertised as a Band 4 post, this could also be offered as Band 3 development opportunity.
