This is an exciting opportunity for an experienced self motivated health care support worker to join the Community Dietetic Team.
Under the guidance of the Advanced Dietetic Practitioner, you will support the provision of a dietetic service targeting health inequalities for patients within the Learning Disability Service.
This will focus on improving health outcomes for those in the learning disability population diagnosed with, or at risk of developing, Type 2 Diabetes. Within the remit of the Scottish Governments Type 2 Diabetes Framework, you will have a key responsibility in the delivery of early intervention, using an adaptation of the Live It Highland: Living with Diabetes programme. This education is currently delivered within the Learning Disability Service on a one-to-one basis, and can involve developing and using a range of interactive resources.
An ability to use your initiative and adapt your communication style in a person-centred approach is essential.
There is scope for you to support further development of the toolkit to include training/resources for carers and support staff working with patients with a Learning Disability and Type 2 Diabetes.
This post involves holding a small caseload of patients, for which you will be supported to undertake full nutritional assessments, care planning and monitoring. Patients may be seen within care homes or as domiciliary visits. Some travel throughout the local community should be expected, and a full, clean driving licence required.
The role supports a number of administrative tasks including making appointments, using nutritional analysis programmes, and developing easy read dietary resources. Within the wider Community Dietetic Team, you will participate in regular team meetings, and have access to learning opportunities and support from the friendly Dietetic Team.

NHS Scotland is reducing their full time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
