Full Job Description

Rheumatology is an evolving service, and we are currently looking for an experienced highly motivated Advanced Nurse Specialist to join our multidisciplinary rheumatology team in Forth Valley. 

The successful applicant  will work as an integral member of the rheumatology nursing team providing direct patient care in the outpatient setting. Utilising their expert knowledge base and clinical competence to manage people with complex rheumatology care needs, supporting our team to deliver the best care possible.

The post holder will be required to undertake highly skilled assessment and clinical interventions at various stages for individuals living with this chronic disease process.

They must have the ability to work independently, managing a complex case load, and possess a high level of interpersonal and confident communication skills to support this specific client group whilst managing telephone advice lines / triage of incoming calls, patient referrals, interface with hospital clinicians and general practitioners to maintain excellent clinical links between the services. 

The post holder will act as a resource for expert advice and training for nursing staff, medical staff, other health care professionals and other agencies. The post holder will lead and contribute to issues relating to rheumatology care locally and nationally where appropriate.  

They should have evidence of on-going professional development.

You should have experience working at this level and previous experience working as part of a multi-disciplinary team assessing and implementing care for individuals with complex needs due to long term health conditions.
