Community Clinical Support Workers

Starting Salary:  Step1 £27,933 WTE pro rata, per annum (rising to Step2 £28,998 WTE subject to satisfactory performance review)

Unsocial Hours Shift Allowance + Mileage Allowance + Pension + Benefits

Hospice Band: 3

Hours: 36 hours per week - 12 hour day shifts / 3 days out of 7 / no set pattern.

We are looking for new team members to join our Care at Home Team. The successful candidates will have the opportunity to be involved in the shaping and development of the service.

The Care at Home team provides practical, social and emotional support to palliative patients and their families within their own homes. The aim of the service is to offer:

1. A person centred approach in the prevention of admission to or facilitate discharge from inpatient care, crisis management and support at times of rapid change including end of life care

2. Respite support for families and carers. The promotion of independence through enabling, empowering and educating patients and families.

The Care at Home service is a 7-day service providing support from 8am-8.30pm. The team has a registered Nursing staff Lead and Co-coordinator (office/home based) and a Community Clinical Support Worker team (CCSW), covering North Edinburgh and East Lothian. A full UK driving license and use of a car is required for this role.
