We are looking to recruit a Band 6 CPN to our growing service within the enhanced community dementia team.
The service is currently in an exciting period of growth and transformation, to provide locality based specialist dementia care for pre, peri and post diagnostic dementia care and support.
This is an interesting and exciting opportunity that will provide you with a challenging and rewarding role within the multi professional dementia team and the opportunity to be part of shaping this developing service.
We will expect you to have well-developed clinical assessment skills, leadership skills, an ability to compile care plans in partnership with service users and carers.
The priorities of this post will be to facilitate rapid access to a diagnosis, treatment, crisis prevention and intervention along with preventing avoidable hospital admissions and assisting with timely discharge from hospital.
The service comprises of three teams staffed with mental health nurses, occupational therapists, social workers, senior health care support workers and Alzheimer Scotland post diagnostic link workers.
All of the teams are an integral part of the Argyll and Bute Enhanced Community Dementia service and work jointly with Community Health Services, Mental Health, Primary Care and other agencies including social care.

NHS Scotland is reducing their full time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay. This reduction will also be applied pro rata for part time staff. This advert and any subsequent offer/contract of employment therefore reflects the new working hours. However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.
