Full Job Description

NHS Greater Glasgow and Clyde is one of the largest healthcare systems in the UK employing around 40,000 staff in a wide range of clinical and non-clinical professions and job roles. We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.

The shift pattern for this position is 37 hours over 7 days.

Role Overview

We are seeking a dedicated and compassionate District Nurse to join our team at West Dunbartonshire Health & Social Care Partnership (HSCP). You will provide person-centred care to patients in their homes or local settings, promoting independence and enhancing quality of life.

You will work closely with a multidisciplinary team, delivering holistic care and supporting patients with a variety of complex needs, including chronic conditions, palliative care, wound management, and rehabilitation.

Key Responsibilities

  • Provide high-quality nursing care to individuals in the community, ensuring the needs of patients are assessed and met.
  • Lead and manage a small team of community healthcare professionals.
  • Work collaboratively with GPs, social care, and other allied health professionals to deliver integrated care.
  • Support patients and their families with education and self-management strategies.
  • Promote health and wellbeing through proactive intervention and health education.
  • Maintain accurate and up-to-date patient records.

Essential Requirements

  • Registered Nurse (Adult) with valid NMC registration.
  • District Nurse qualification (Specialist Practitioner Qualification) or equivalent experience.
  • Experience in community nursing and delivering care to patients with complex needs.
  • Strong leadership skills and the ability to manage and support a team.
  • Excellent communication and interpersonal skills.
  • Full UK driving licence and access to a car for work purposes.
