Full Job Description

The shift pattern for this position is Monday to Sunday.
As part of a multidisciplinary team the post holder will have responsibility for delivering Comprehensive Geriatric Assessment (CGA) to older adults on admission to the RAH. The role will facilitate CGA and provide Liaison throughout the hospital. This role will support safe onward transfer and supported discharge of patients through appropriate screening and patient/family/carer involvement.
The post holders will act as an expert resource providing advice where required to members of wider multidisciplinary team.
