East Lothian Hospital to Home Team

East Lothian Hospital to Home team have exciting opportunities for Health care Support Workers to join their team.  

Our team is expanding, and we are looking for caring, compassionate and motivated individuals to bring influential change and help develop our service to assist people who are ready for home; facilitating early discharge from hospital.

The East Lothian Hospital to Home team, works in partnership with the H&SCP locality teams and in collaboration with NHS Lothian, Primary Care, Voluntary and Third Sector services to support people to stay longer at home. 

The role involves assessing, planning, providing, managing, monitoring and evaluating nursing care, following best practice to meet the requirements of people with nursing needs in a community setting.  Your responsibilities will include assistance with activities of daily living, medication, meal prep, end of life care and palliative care.
