Full Job Description
- The post holder must possess a full driving licence and provide a car for business use.
- Alternative travel arrangements will be discussed with disabled candidates.
- Willing to work flexibly to ensure proper discharge of duties.
- Willingness to participate in training opportunities.
- Ability to work from home on a regular basis, with the requirement to attend your primary work base as and when required.

- Good general education to GCSE level or equivalent including English Language and Maths (A-C),
- Grades 7-9 are equivalent to the old grades A and A.
- The bottom of grade 7 aligns with the bottom of grade A.
- Grades 4-6 are equivalent to the old grades B and C.
- The bottom of grade 4 aligns with the bottom of grade C, Evidence of interest and aptitude for work with older people, people with physical disabilities, adults with learning disabilities and sensory impairment and their carers and/or related work,
- Informal carer experience, so understand the issues that can arise for people with caring responsibilities.
- Experience of working in a health setting
- Experience of working with staff from different professional backgrounds. Specialist skills and abilities Method of Assessment - D, IEssential:
- Demonstrate an understanding of the needs of older people, people with physical disabilities, adults with learning disabilities and sensory impairment and their carers.Desirable:
- Knowledge of the 2014 Care Act
- Knowledge of self-directed care, personal budgets, reablement
- Knowledge of hospital discharge processes
