Full Job Description

An exciting opportunity has arisen for an innovative individual to work as a Patient Navigator within the GI cancer Colorectal nursing service, within the Surgical Directorate. We are seeking an individual who will embrace this exciting and rewarding role.

You will be an integral part of the Colorectal cancer nursing team and will assist, support and co-ordinate the pathway for their patients. As a patient navigator in this service, you will be involved with patients on a suspected or confirmed colorectal pathway, supporting them throughout their cancer pathway by maintaining high patient experience and outcomes.

You will ensure the efficient channelling and close coordination of the patient pathway ensuring robust communication between patients, relatives, nurses and oncology teams. You will signpost patients and carers to the relevant source of support and information appropriate to their needs.

You will demonstrate excellent organisational skills, be flexible in approach, able to exercise initiative and demonstrate a consistently high standard of professionalism, being aware of the need for confidentiality and integrity.

A requirement of this post is to become a member of the Protection of Vulnerable Groups (PVG) Scheme Prior to appointment.

NHS Scotland is reducing their full-time working week from 37.5 to 37 hours per week from 1 April 2024 but with no change in pay.  This reduction will also be applied pro rata for part time staff.  This advert and any subsequent offer/contract of employment therefore reflects the new working hours.  However, as not all service areas will be able to adopt the 37 hour working week immediately from 1 April 2024, you may be required to work up to an additional 30 minutes per week for a temporary period for which you would be paid until the service you are working in changes rosters or working patterns to accommodate the new reduced working week.  If you have any questions or concerns, please contact the Recruiting Board.
