As a Personal Health and Wellbeing Practitioner, you'll play a vital role in helping individuals on their journey to recovery, independence, and wellbeing. Every day, you'll provide meaningful support and guidance to those with complex needs, helping them unlock their potential and achieve their goals. Aspinden Care Home (ACH) is a CQC registered care home based in Bermondsey, South East London. It has a national remit and cares for those with complex needs and/or those with long term alcohol addiction. ACH offers residents the chance to gain stability in their lives and improve their physical health and mental well-being. Shift Pattern: 37.5 hours per week, Days and nights, 12.5-hour shifts on a rolling rota, including weekends, We are an inclusive employer, committed to Equity, Diversity, and Inclusion (EDI). - Experience of working with people of complex backgrounds and/or a good understanding of the sector
- Ability to use, learn and adapt to IT at an intermediate level, including Microsoft and other software programs
- Ability and willingness to show flexibility of working patterns, responding to the needs of the service and residents
- Ability to promote the service and provide outreach-based provision, with an ability to liaise and work effectively in partnership with stakeholders
