An exciting opportunity has arisen for a UK Registered Nurses to join the Care Home Team during a period of ongoing development under Dundee Health and Social Care Partnership.    

The successful applicant will be joining an integrated Care Home Team consisting of general nurses, mental health nurses, social work review officers and a specialist occupational therapist.  Each care home in Dundee has a dedicated Link UK RMN, UK RGN and Review Officer to support residents with all aspects of their health and wellbeing whilst also ensuring their safeguarding against harm.  Advice, guidance, education and specialist support is offered to all care home staff to ensure that they maintain a good standard of holistic care and that the immediate and enduring needs of all residents are being met. The job description attached for this vacancy does not fully reflect the role due to the evolving nature of this position. 

The main objectives for the UK RGNs within the team are to prevent admission from care home into hospital, including delayed discharges from hospital to care home; to support the care homes to meet the changing needs of residents due to chronic disease and increased frailty and how to identify the deteriorating resident using a variety of assessment tools; to offer training/awareness sessions for all care home staff to support the delivery of evidenced-based practice and support with the learning and development of clinical skill based tasks; to conduct Future Care Planning (FCP) and Do Not Attempt Cardio Pulmonary Resuscitation (DNACPR) following in depth discussions with residents and/or their legal proxy; to understand and practice within the Adult Support and Protection (Scotland) Act 2007 and attend/support homes through LSI (Large Scale Investigations) as and when required.

The successful applicant joining the team must be able to work independently, under the support and guidance of the RGN Charge Nurse and Team Manager.  There will be a requirement to construct links with other statutory and non-statutory agencies as well as engage with the public to enhance and improve the service.  The post holder is responsible for assisting with the promotion and ongoing development of general health support within care homes.  The successful applicant will hold a dedicated caseload, work as an independent practitioner and provide education and training to care home staff.  As a member of a multi-disciplinary team, the post holder will have excellent communication, written and negotiations skills in promoting a person-centred approach when working with older people with physical health problems.

The post holder will be working 09:00-17:00 Monday to Friday.

Hours of work are: 37 hours per week.
