An exciting opportunity has arisen within the Caithness Health & Social Care team for motivated and enthusiastic individuals, who will be part of a multidisciplinary team working with people in their own homes to provide holistic care.

Your primary responsibilities would will be to assist the integrated teams in facilitating early hospital discharges and ensuring a smooth and person centred transition from hospital to home; supporting Allied Health Professionals (AHPs) to assess people for services in their own homes, not in a clinical setting and to provide a period of reablement care and support to help people gain back their independence following a hospital admission or period of ill health. The post holders will need to be flexible, able to use their own initiative and will provide high standards of care and support, assisting the team in delivering a high quality service to the local community.

Experience not required, as full training and induction will be given. The post holders will have the opportunity to complete their SVQ in Health & Social Care and the possibility of further career progression.

A driving licence is essential for this post and applicants should have access to their own vehicle for work purposes, travel expenses available. There are pool vehicles available in rural areas.

The hours will be 4pm 9pm, worked on a rota basis of four days on and four days off. Enhancements are paid for unsociable hours and weekend work.

The service works across Caithness, with bases in Wick and Thurso. Additional hours will also be available through the integrated staff bank.
