Assessment and Care Management Team – Physical Disability Team

The successful candidate will be based within the physical disability team undertaking a range of social work services in relation to the assessment and care management of adults with physical disability in West Lothian. Candidates should ideally have experience of working with this client group.

The post holders will be assisting with care pathways, enabling people to remain as independent as possible. Assessment will take a strengths-based approach and the post holders will be encouraged to think creatively about how we support service users during their period of assessment. This will include ensuring service users meet eligibility criteria.

The successful candidate will have an ability to work as part of a team, respond to demands of a busy service, be able to follow procedure, have excellent communication and report writing skills. Tasks will be carried out in partnership with service users, family members, social work colleagues and other professionals and agencies.

The candidate will be required to have a good understanding of Self-Directed Support and the importance of completing outcome focused assessments as well as Adult Support & Protection legislation

A professional Social Work qualification is essential.

This post is considered Regulated Work with children and/or protected adults under the Protection of Vulnerable Groups (PVG) Scotland Act 2007. From 28 February 2011 preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council
