The team consists of a team leaders, 2 senior practitioners, 2.5 social workers and 1.5 family support workers and has dedicated administrative support. The care at home crisis has resulted in some creative and innovative practice being developed, with the team undertaking all aspects of this work from referral, assessment, setting up and reviewing packages of support.
