Full Job Description

Based in Integrated Discharge Team at Ninewells Hospital and/or Royal Victoria Hospital, you will work full time, 37 hours per week., You will provide a high quality social work service to hospital in-patients to support and facilitate hospital discharge, in accordance with local and national policy and guidance.

You will ensure all statutory requirements are met in respect of The Social Work (Scotland) Act 1968, The Adult Support and Protection (Scotland) Act 2007, The National Health Service and Community Care Act 1990 and The Community Care and Health (Scotland) Act 2002.

You will have a Social Work Degree, CQSW, Dip SW, CSS (with appropriate option)., You will have proven post-qualifying experience in a social work role, experience of working in a multi-disciplinary setting and have commitment to social work values/anti-discriminatory practice.

You will have excellent communication skills, the ability to prioritise and to work autonomously and have assessment skills. You will have ability to be good team player, have flexible approach and strong sense of social work identity.

You will have the ability to be registered with the Scottish Social Services Council within a specified time., Disability Confident
