Full Job Description

Social Worker required within the Integrated Community Assessment Team based at the Jeanie Deans Unit in Helensburgh. The main function of the post is to carry an Adult Caseload and provide the full range of Assessments and Care Management functions for clients/patients being discharged from hospital back to the local community. You should be able to provide evidence of previous operational management skills in Adult Care Social Work and be skilled in Adult Protection practice.
