The government funded programme delivers inclusive voluntary throughcare services across Scotland for men, women and young people serving a short-term sentence or following a period of remand. About your key skills and experience:
- Trauma-Informed Support: Knowledge and practical experience in delivering trauma-informed care and person-centred support.
- Building Relationships: Strong interpersonal skills with the ability to build trust and rapport with individuals from diverse backgrounds.
- Collaborative Working: Experience working in multi-agency teams, with a focus on partnership building and supporting individuals through complex needs.
- Risk and Needs Assessments: Ability to conduct thorough assessments and provide effective risk management and safeguarding strategies.
- Case Management: Experience in maintaining accurate case records, including completion of assessments, progress reports, and action plans.
- Flexibility and Adaptability: Comfortable working across various cohorts and regional areas, adapting to changing service demands as needed.
- Communication: Excellent verbal and written communication skills, with a focus on clear, empathetic, and professional interactions.
About the Role:
We are seeking compassionate and dedicated Community Workers to join our dynamic and collaborative team working across Argyll & Bute, East Dunbartonshire, West Dunbartonshire, East Renfrewshire, Renfrewshire, Inverclyde, committed to providing trauma-informed, person-centred support to individuals leaving prison. This role plays a vital part in supporting people through their journey of reintegration into the community. You will be part of a national initiative involving eight charitable organisations, led by Sacro, working in partnership with the Scottish Prison Service and local agencies. The goal is to reduce re-offending and improve wellbeing through effective, personalised support.
A driving license and access to a vehicle for business use is essential for this role.
Responsibilities:
- Demonstrate commitment to establishing trusting and genuine relationships, rooted in trauma informed principles
- Work collaboratively with the partnership team to deliver relational support for people accessing the NVTP
- Support and deliver effective pre-release planning
- Contribute to multi-agency meetings
- Deliver gate liberation support at this critical juncture
- Complete robust and dynamic risk assessments and safeguarding practice
- Conduct suitable needs assessment tools, tailored to the individual
- Complete accurate and timely records on the case management system to maintain high quality service standards
- Deliver assertive outreach to individuals to maximise engagement
- Identify suitable onward referrals at the earliest appropriate point
- Enable mutually agreed and gradual endings to support provision
- Actively participate in partnership meetings to support team working and service improvement
- Adherence with role training (including requirements of NVT service and organisation training)
- Flexible and responsive to service demand and willingness to work across cohorts and regional areas as required
