The team work extremely well together to ensure the safety of vulnerable adults under the close supervision of a supportive Team Manager The responsibilities will include:

• Support reviewing hospital discharges
• Working from home to conduct Care Act assessments, Support Pans and Reviews
• Must be knowledgeable regarding the new hospital discharge requirements under scheme 1 and Scheme 2 funding streams
• Have experience of NHS Continuing Health Care Assessments
• Mental Capacity Act assessments
• Liaise with other professionals
• 36 hours per week (flexible by negotiation)
• 2 Month Temporary Contract
• Start date subject to DBS and reference clearance
